WHY?
A teacher explains WHY her class will be using portfolios in her class:

What Goes Inside?
Ideas of things to include in all portfolios:
- An About Me or Biography section
- A mission or purpose statement for the portfolio
- Reflection on all processes/products created for the portfolio
- If a showcase portfolio, include writings on why each piece was selected for inclusion, the skills demonstrated, and the standards mastery demonstrated by the piece
- Reflection on the portfolio process (usually at the end of the portfolio process)
- A mix of media - text, audio, images, video
- Proper credit for all images/quotes/work that do no belong to the student (works cited/bibliography/credits page)
- A personalized header (school appropriate)
- A personalized profile picture (school appropriate)
Ideas for portfolio content by content area:
ELA
- Writing samples from different genres
- Brainstorming/mindmapping/outlining/pre-writing
- Reflections on changes in writing due to peer/teacher/audience feedback
- Story summaries
- Vocabulary (could be done with text, images, video, etc.)
- Class notes
- Student-created test review ideas (games, presentations, digital flash cards, etc.)
- Character analysis (or projects like fake facebook walls, etc.)
- Images of non-digital projects with explanations of the project, skills mastered, reasons for inclusion in portfolio, etc.
Math
- Class notes
- Sample problems
- Student-created videos explaining a concept/solving a problem to demonstrate mastery
- Writing about real-world math
- Challenge problem solving (given challenge math problems by teacher and complete them individually or as a team for extra points - putting all work and reasoning in the portfolio)
- Collect data on items missed and reflect on why they were missed
- Link to student-found tutorials to help with student-identified (or teacher-identified) weak areas
- Vocabulary
- Student-created test review ideas (games, presentations, digital flash cards, etc.)
- Images of non-digital projects with explanations of the project, skills mastered, reasons for inclusion in portfolio, etc.
Science
- Class notes
- Lab reports with photos and video to augment text
- Writing about science in real life
- Challenge problem solving (given challenge science scenarios by teacher and complete them individually or as a team for extra points - putting all work and reasoning in theportfolio)
- Vocabulary
- Lab safety rules (again with photos or videos)
- Link to student-found tutorials to help with student-identified (or teacher-identified) weak areas

Social Studies

- Historical Figure Biographies (or projects like fake facebook walls, etc.)
- Images of non-digital projects with explanations of the project, skills mastered, reasons for inclusion in portfolio, etc.
Fine Arts
- Audio recordings for personal/peer/teacher review and feedback
- Video recordings for personal/peer/teacher review and feedback
- Concept photo galleries for theatre sets/costuming
- Written character analysis or character diaries for theatre
- Art process or showcase galleries with reflections
- Video of dance moves for study/critique
- Student-created how-to videos to demonstrate correct technique for others/demonstrate student mastery of techniques

Examples of some Portfolio Rubrics:
You can have rubrics for each artifact and a final rubric for the finished product...or whatever combination works best for your class.
Many suggest that students be given a portfolio planning sheet created by the instructor that list the learning targets to be included in the portfolio, and a place for students to write in the artifacts they will include to demonstrate mastery of the learning targets.
